Use this online form to submit an item to the eWeekly, PCUUC's Thursday e-newsletter. The deadline for submissions is 5 p.m. on Tuesday.

Direct Debit Program Form

Would you like to donate to PCUUC automatically each month? Use this form to have your monthly pledge withdrawn from your bank account.

Congregant Care Fund

Return the completed application and copies of receipts to the CCF Trustee, Sean Smetana. Place in the collection basket in an envelope or mail to PCUUC, attn: CCF Trustee, P.O. Box 858, Grayslake, IL 60030.

Tax Exempt Certificate

Print this certificate and use it when you purchase budgeted items on behalf of PCUUC. Please put PCUUC items on a separate receipt.

Reimbursement Form

Use this form to file for reimbursement after you've purchased budgeted items on behalf of PCUUC. Signatures from a Committee Chair and a Board Member are required.

Conflict Resolution Form

In the case of a conflict within PCUUC, Use this form to ask for additional support to facilitate a resolution between the parties. Click here for the Covenant of Right Relations Conflict Resolution Policy.
